| The most important thing you can do is to
report your accident to us as soon as possible. Fax us a copy of your accident
report at 212/269-6212. If you have questions please call us at 212/709-8600. If the injury or work-related illness requires the worker to seek medical attention on three or more visits to a medical provider, or he/she misses two or more days of work as a result of the incident, then you must file a C-2 Form with the Workers’ Compensation Board. We will send a copy of your accident report to the insurance carrier and facilitate claims processing. |
